After powering down the device in Android or Ubuntu I notice the next set of power ups (using the PWRKEY) result in failed boots as shown below:

After a number of random attempts the board does finally boot.

Please try a long press of PWR button (>3s) to see if the randomness will go away.

Another have question, what the maximum voltage the ADC input pins can take?

The maximum voltage the ADC input pins can take is 1.8V .
